Notice and Consent Form for Collection of Personal Data

The Industrial Development Administration, Ministry of Economic Affairs (hereinafter referred to as "IDA"), has commissioned the Taipei Computer Association to carry out The Plan for Space Industry Supply Chain Development and Communications Industry Talent Empowerment (hereinafter referred to as "the Plan"). Taipei Computer Association will collect your personal data. In accordance with the Personal Data Protection Act and related regulations on personal data protection, we hereby inform you of the following matters before collecting your personal data. By checking "I agree," you indicate that you have read, understood, and agreed to accept all the contents of this consent form.

1. Collection Purpose and Categories
   For the purposes of conducting or executing relevant operations, activities, projects, providing services, and for internal administrative management of the association, as well as for reporting to the competent authority or other purposes prescribed by the association's business scope, and for sending information about the association or industry-related activities.
2. Personal Data Utilization Period, Region, Recipients, and Methods
   Your personal data, unless involving international business or activities, will be used by the relevant agency(ies) within the territory of the Republic of China, within the necessary and reasonable scope for the above-mentioned collection purposes until the disappearance of the said collection purposes.
3. Exercise of Data Subject Rights
   In accordance with Article 3 of the Personal Data Protection Act, you may request the execution unit of the project to inquire about or access, provide copies of, supplement or correct, stop collecting/processing/utilizing, or delete your personal data.
4. Impact of Refusal to Provide Personal Data
   If you do not provide or provide incorrect personal data, or request to stop collecting/processing/utilizing/deleting personal data, or unsubscribe from service messages, the agency(ies) will not be able to provide you with relevant services for the collection purposes.

Personal Data Consent:

2. I understand that this consent form complies with the Personal Data Protection Act and relevant regulations, and I agree to provide it to the Utilizing Party for retention and future verification purposes.
